What it is

Therapeutic regimens or combination treatments that combine the short-acting SSRI dapoxetine with a phosphodiesterase type-5 (PDE5) inhibitor (such as sildenafil, tadalafil, or vardenafil) to address co-existing premature ejaculation (PE) and erectile dysfunction (ED).

How it works

Dapoxetine works by inhibiting presynaptic serotonin reuptake to increase serotonin activity in the synaptic cleft, delaying the ejaculatory reflex. The PDE5 inhibitor works locally in the penile tissue by preventing the breakdown of cGMP, relaxing vascular smooth muscle and increasing blood flow during physical sexual stimulation to help achieve and sustain an erection.

Typical use

Although combined therapies have been evaluated in clinical studies, the manufacturer summary of product characteristics (SPC) explicitly states that dapoxetine should not be used in men with ED who are using PDE5 inhibitors due to an increased risk of orthostatic hypotension and syncope. If medically approved under strict clinical supervision, they are taken on-demand before anticipated sexual activity and must never exceed a single dose within a 24-hour period.

Onset & duration

Dapoxetine has an onset of 1 to 2 hours and rapid initial clearance. Co-administered PDE5 inhibitors typically begin working within 30 to 60 minutes; the duration of erectile support ranges from 4 hours (sildenafil) to up to 36 hours (tadalafil).

Common side effects

Headache, nausea, dizziness, flushing, indigestion, heartburn, nasal congestion, diarrhoea, back pain, and muscle aches.

Serious warnings

Concomitant use significantly elevates the risk of orthostatic hypotension (sudden blood pressure drop upon standing) and vasovagal-mediated syncope (fainting), which can cause serious falls or accidental injury. There is a serious risk of priapism (a painful erection lasting longer than 4 hours), which is a urological emergency. Stop use immediately and seek clinical help for sudden vision or hearing loss.

Who should avoid

Do not take if you use nitrates, nitric oxide donors (such as recreational poppers), or riociguat. Avoid if you have significant cardiovascular disease (heart failure, unstable angina, recent heart attack/stroke, or advised against sexual activity due to heart strain). Strictly contraindicated in patients with a history of syncope, postural hypotension, mania, or severe depression. Contraindicated in moderate or severe hepatic impairment and severe renal impairment. Not for use in women, children, or men aged 65 and over.

Interactions

Nitrates and riociguat (absolutely contraindicated). Alcohol must be avoided, as it significantly increases the risk of syncope, somnolence, and slow reflexes. Do not combine with recreational drugs. Do not consume grapefruit juice within 24 hours of taking dapoxetine. Multiple interactions occur with strong CYP3A4 inhibitors (contraindicated), moderate CYP3A4 inhibitors (clarithromycin, erythromycin, fluconazole), potent CYP2D6 inhibitors, alpha-blockers, and anticoagulants.
