What it is
An FDA-approved phosphodiesterase type-5 (PDE5) inhibitor indicated for treating erectile dysfunction (ED) to help men restore physical intimacy.
How it works
It acts by regulating the PDE5 enzyme, allowing the smooth muscles and blood vessels in the penis to relax so that increased blood flow can support an erection. Note that it requires natural physical stimulation to work and does not automatically trigger an erection.
Typical use
Taken on an as-needed basis prior to sexual activity. It is not a cure for ED, does not increase libido, and does not prevent pregnancy or protect against sexually transmitted diseases like HIV.
Onset & duration
Usually begins working within 30 minutes to 1 hour. Effects for erectile dysfunction typically last up to about 4 hours (often less after 2 hours). Do not take more than one dose in 24 hours. A high-fat meal may delay onset.
Common side effects
Headaches, facial flushing, indigestion, heartburn, diarrhea, nausea, nasal congestion, muscle aches, back pain, and temporary changes in color vision.
Serious warnings
Seek emergency medical care immediately for an erection lasting longer than 4 hours (priapism) to prevent permanent tissue damage. Discontinue use and contact a doctor instantly for sudden severe vision or hearing loss, chest pain, or fainting.
Who should avoid
Do not use if you are taking nitrate medications, riociguat, or recreational poppers (amyl nitrate). Avoid if you have been advised by a physician to avoid sexual activity due to cardiovascular strains. Contraindicated in individuals with severe liver or kidney disease, low blood pressure, uncontrolled high blood pressure, leukemia, multiple myeloma, sickle cell anemia, or a curved penis.
Interactions
Nitrates, riociguat, alpha-blockers, blood pressure drugs, certain antibiotics (like erythromycin and clarithromycin), HIV protease inhibitors, azole antifungals, St. John's Wort, grapefruit, and alcohol.
