What it is
A short-acting selective serotonin reuptake inhibitor (SSRI) licensed in the UK and Europe for the on-demand treatment of premature ejaculation (PE) in adult men aged 18 to 64 years.
How it works
It inhibits the neuronal reuptake of serotonin, subsequently potentiating the neurotransmitter's action at pre- and postsynaptic receptors to increase ejaculatory latency and improve control over ejaculation.
Typical use
Taken strictly as an on-demand therapy approximately 1 to 3 hours prior to anticipated sexual activity. The recommended starting dose is 30 mg, which may be increased by a clinician to a maximum of 60 mg if the initial response is insufficient and no moderate to severe adverse reactions or syncopal symptoms are experienced. The tablet must be swallowed whole with at least one full glass of water to avoid a bitter taste and reduce the risk of fainting. It must not be taken more than once every 24 hours.
Onset & duration
Dapoxetine is rapidly absorbed, reaching peak plasma concentration (Cmax) approximately 1 to 2 hours after oral administration. It is cleared rapidly, with plasma levels dropping to less than 5% of peak concentration within 24 hours post-dose. It has an initial disposition half-life of 1.5 hours and a terminal half-life of approximately 19 hours.
Common side effects
Very common side effects (affecting more than 1 in 10 men) include dizziness, headache, and nausea. Common side effects (affecting up to 1 in 10 men) include anxiety, agitation, restlessness, insomnia, abnormal dreams, decreased libido, somnolence, tremor, paraesthesia, blurred vision, tinnitus, flushing, sinus congestion, yawning, diarrhoea, vomiting, constipation, abdominal pain, dyspepsia, dry mouth, erectile dysfunction, and fatigue.
Serious warnings
May cause vasovagal-mediated (neurocardiogenic) syncope (sudden loss of consciousness), which typically occurs within 3 hours after dosing and may be preceded by prodromal symptoms such as nausea, lightheadedness, and sweating. Orthostatic hypotension (a significant drop in blood pressure upon standing) can occur. SSRIs have been associated with an increased risk of suicidal ideation and mood changes, as well as a lowered seizure threshold. Caution is advised for patients with bleeding or clotting disorders.
Who should avoid
Contraindicated in patients with significant pathological cardiac conditions (such as heart failure NYHA Class II-IV, conduction abnormalities, ischemic heart disease, significant valvular disease, or a history of syncope/postural hypotension). Contraindicated in individuals with a history of mania, bipolar disorder, severe depression, uncontrolled epilepsy, or moderate to severe hepatic impairment. Not recommended in patients with severe renal impairment (CrCL < 30 mL/min). Strictly contraindicated with concomitant MAOIs, thioridazine, other SSRIs/SNRIs/TCAs, or other serotonergic products (wait 14 days after stopping them; wait 7 days after stopping dapoxetine). Contraindicated with potent CYP3A4 inhibitors. Not indicated for women, children, or men aged 65 and over.
Interactions
Do not combine with alcohol, as it enhances neurocardiogenic adverse events such as syncope and increases the risk of accidental injury. Avoid recreational drugs (serotonergic substances like MDMA/LSD risk serious reactions like serotonin syndrome; sedatives increase dizziness). Avoid grapefruit juice within 24 hours prior to dosing. Concomitant use with PDE5 inhibitors (such as sildenafil, tadalafil, vardenafil) is contraindicated due to possible reduced orthostatic tolerance. Dose is restricted to 30 mg with moderate CYP3A4 inhibitors (e.g., erythromycin, clarithromycin, fluconazole, verapamil, diltiazem). Use with caution with potent CYP2D6 inhibitors, vasodilators, or anticoagulants/antiplatelet medications.
